Interesting config file items include:
ihtmlheaderfile = indexheader.html
ihtmlfooterfile = indexfooter.html
mhtmlheaderfile = mailheader.html
mhtmlfooterfile = mailfooter.html
(the file names are my example ones, update to fit your own system)
