Found the no-index file problem

Daniel was correct. There was a problem and I created it.

Changelog info:
 - Error in setting of defaultindex in configure.in caused no index.html
   file to be created. configure.in was corrected to set the default
   index to "thread" instead of thread.html.

If you used a .rc file with defaultindex set then this did not show up.
Guess how I was testing it. If you did not set a defaultindex value
then it used the default, The default was being incorrectly set in
configure.in to thread.html. thread.html never matched "thread",
"subject", "author" or "date" so no index file default was being set.

It has been corrected, a new configure re-generated and baselined.
